Comprehensive primary care for adults including annual wellness exams, preventive screenings, chronic disease management, and acute illness care. We serve parents alongside their children.

Chronic Disease Management

Coordinated care for diabetes, hypertension, asthma, obesity, and other chronic conditions. We address the mental health and lifestyle factors that affect disease management.

Diabetes care Blood sugar monitoring, A1C tracking, medication management, nutrition counseling, and complication prevention strategies.
Hypertension Blood pressure monitoring, lifestyle modifications, medication optimization, and cardiovascular risk reduction.

Preventive Health

Acute illness care when you need it. We reserve appointments for same-day concerns so you don't have to go to urgent care or wait while sick.

Same-day access Reserved appointment slots each day for urgent concerns—call in the morning and be seen that afternoon.
Acute illness Treatment for colds, flu, strep throat, UTIs, sinus infections, stomach bugs, and other sudden illnesses.
Minor injuries Care for sprains, strains, minor cuts, burns, and other injuries that don't require emergency services.
Telehealth option Video visits available for many acute concerns—get care from home when you're too sick to travel.

Integrated Behavioral Health

Mental health screening at every visit with same-day consultation available. Your primary care provider works directly with our behavioral health team.

Same-day consultation If your provider identifies a concern, our behavioral health team can meet with you that same day.

Can I see the same provider each visit?

Absolutely. We believe continuity of care is essential and work to ensure you see your preferred provider. You'll build a relationship with someone who knows your history.

Do you provide telehealth visits?

Yes, we offer telehealth for follow-ups, medication refills, and many acute concerns. In-person visits are available for physical exams and procedures.

What ages do you see?

We see patients of all ages—from keiki to kūpuna. Families can schedule together, and we can address multiple generations' health needs in coordinated care.
